Procedures for University Departments Disposing of University Property All equipment that a University entity wishes to dispose of must be sent to Surplus Property. To have items approved as surplus and picked up by Surplus Property, please fill out an Equipment Accountability Change Request Form (EACR) (Note: This is a PDF file which requires Adobe Reader 6.0 or higher.) Please refer to the EACR Instructions, or contact Surplus Property at 491-1199, for directions on how to properly fill out this form. Completed EACR forms must be sent to Property Management for approval. Do not send EACR forms directly to Surplus Property. Certain items have special handling, recycling or disposal requirements. Fees will be assessed to departments for disposition of these items. For more information please refer to the Surplus Property Service Fees webpage. Certain items such as dry or liquid chemicals, items containing mercury, any unknown substance, or any equipment that has been exposed to such contaminants are considered hazardous. Surplus Property cannot pick up or accept these items.
